Every year, Medicare evaluates plans based on a 5-star rating system. Devoted Health’s HMO Contract H2697 earned 5 out of 5 stars, H1290 and H7993 earned 4.5 out of 5 stars, and H8173 earned 4 out of 5 stars in 2023.

By submitting your information, you give Devoted Health or our designee permission to contact you to discuss plan options (auto-dialer, text, or email) for up to a year from the date we receive your response and you agree to our terms and conditions. Cellular carrier charges and data rates may apply. Text frequency from Devoted Health varies. You can opt out at any time by calling 1-800-338-6833 (TTY 711). By text message, you can opt out by replying STOP. You can also reply HELP to get help. Opting out will not impact your ability to enroll or receive services.

Devoted Health is an HMO and PPO plan with a Medicare contract. Our D-SNPs also have contracts with State Medicaid programs. Enrollment in our plans depends on contract renewal. To enroll in a Devoted Health plan you must meet certain eligibility requirements and reside in the plan's CMS-approved service area. Our plans' 2023 service areas, as applicable, include select counties in Alabama, Arizona, Colorado, Florida, Hawaii, Illinois, North Carolina, Ohio, Oregon, Pennsylvania, South Carolina, Tennessee, and Texas.
